Ebola outbreak in DR Congo could reach West Africa epidemic scale, CDC modelling shows
US modelling suggests that Ebola transmission in Central Africa could escalate without stronger isolation and containment measures in place
The US Centers for Disease Control and Prevention (CDC) has warned that the Ebola outbreak in the Democratic Republic of the Congo (DRC) could develop on a trajectory approaching the scale of the 2014–16 West Africa epidemic if containment efforts remain insufficient.
In their analysis, CDC modellers set out a range of scenarios estimating >10,000 cases, depending largely on how quickly infected individuals were identified and isolated.
The West Africa epidemic, the largest recorded, resulted in more than 28,000 cases and over 11,000 deaths between 2014 and 2016.
At the time of the modelling, health authorities had reported about 400 confirmed cases and 63 deaths in the DRC. However, officials acknowledged that under-reporting was likely due to surveillance gaps and delayed diagnosis.
The outbreak involves the Bundibugyo strain of Ebola virus, for which no approved specific treatments or vaccines are available, heightening concern among health and assistance providers operating in the region.
The World Health Organization (WHO) had previously declared the outbreak a public health emergency in May, as transmission continued in areas affected by armed conflict and population displacement.
The CDC modelling indicates that outcomes are highly sensitive to isolation rates. Scenarios in which 50–70% of cases were rapidly isolated projected totals up to 10,000 infections, while weaker containment capacity was associated with substantially higher case counts.
However, experts cautioned that outbreak modelling carried significant uncertainty, particularly early in epidemics when surveillance data was incomplete. Previous CDC projections during the West Africa outbreak overstated worst-case scenarios, underscoring the difficulty of forecasting transmission dynamics in real time.
The current situation highlights potential risks linked to medical evacuation constraints, claims exposure, and operational disruption in regions also affected by insecurity, including conflict involving the M23 rebel group and attacks by the Islamic State-affiliated Allied Democratic Forces, which has contributed to displacement and hampered response operations.
