Dr Mohammed Afsal, Medical Director at Bluedot Air Ambulance, talks to ITIJ about emerging markets, skilled crew, and the power of partnerships

What unique clinical advantages do air ambulance services bring to patient care in emerging markets, particularly in timecritical interventions like trauma and cardiac emergencies?

Air ambulances bring the hospital to the patient when every minute counts. In trauma or cardiac emergencies, time lost in reaching definitive care can mean the difference between survival and loss. Air ambulances equipped as flying ICUs bridge that gap – offering advanced monitoring, ventilatory support, and critical care expertise en route.

In emerging markets, where access to specialised hospitals may be limited, this capability is transformative. It allows patients in remote regions to receive the same standard of intensive care they would get in a modern ICU, but thousands of feet above ground. At Bluedot Air Ambulance, this clinical capability is supported by a 24/7 control centre staffed with dedicated case managers who coordinate every mission – ensuring medical, logistical, and operational elements align seamlessly. The result is faster responses, smoother coordination, and safer patient outcomes.

What are the biggest challenges in building trained medical and aviation teams for air ambulance operations in India and the Middle East, and how can these be addressed sustainably?

The biggest challenge is building teams that are equally skilled in clinical care and aeromedical transfers. This work demands far more than medical expertise – a deep grasp of flight physiology, seamless coordination of airport and aviation logistics is needed, plus the competence to manage complex prehospital scenarios safely and efficiently.

While India and the Middle East have excellent clinicians, structured training in aeromedical transfers is still evolving. At Bluedot, we’ve created regionally knitted training programmes that amalgamate international best practices with the realities of our regional operating environment.

This approach brings Western standards into a context that’s locally relevant and sustainable. Our induction and simulationbased learning pathways are designed to build confidence and competence in flight doctors, nurses, and paramedics. With continuous education, clear clinical governance, and quality benchmarks such as EURAMI accreditation, we’re developing a generation of aeromedical professionals equipped for both global and regional missions.

How do infrastructure gaps, regulatory frameworks, and cross-border coordination impact the growth of air ambulance services in these regions?

The growth of air ambulance services often depends as much on the ground as in the air. Many airports and hospitals in emerging markets still lack dedicated medical transfer facilities, streamlined customs or immigration processes, and clear pathways for patient movement.

These gaps can delay missions and add complexity to time-critical transfers.

In addition, varying regulations between countries make obtaining medical and aviation clearances a challenge. What’s needed is a harmonised regional framework that allows for faster, safer, and more predictable aeromedical movements.

At Bluedot, we actively collaborate with airport authorities, hospitals, and civil aviation bodies to create pre-coordinated systems that simplify patient transfers across borders – minimising delays while upholding medical integrity.

What innovations or partnerships do you see as critical for scaling air ambulance services in emerging markets, while keeping them affordable and accessible?

The future lies in collaboration and technology. Partnerships between air ambulance providers, commercial airlines, hospitals, and insurers can make patient transfers both accessible and sustainable. Commercial airline medical escort programmes are already demonstrating how this model can provide high-quality care at lower costs.

Innovation also comes through digital transformation. Real-time telemedicine support during flights, artificial intelligence (AI)-assisted triage for mission planning, and centralised patient logistics platforms are redefining efficiency and reach.

At Bluedot, our vision is to build a connected ecosystem linking healthcare, aviation, and technology, so that critical care in transit is not a privilege but a fundamental component of modern healthcare access. By combining clinical excellence with operational intelligence, we aim to make safe, high-quality aeromedical care available to everyone who needs it, wherever they are.
