Flying Doctors Korea describes a recent mission in which it was asked to bring a 65-year-old home from Croatia

A recent mission perfectly encapsulates our operational flexibility and clinical excellence. A 65-year-old female patient suffered a sudden, severe neurological emergency – a ruptured cerebral aneurysm – while travelling in Croatia. Following lifesaving emergency drainage and intensive care at a local hospital, the patient showed signs of stabilisation, allowing the external ventricular drain (EVD) to be safely removed prior to transport.

However, the patient remained highly critical and required extensive life support systems. To manage this, our team deployed a comprehensive mobile intensive care unit (ICU) setup, including a transport ventilator, portable suction unit, multiple infusion and syringe pumps, an advanced patient monitor, and an i-STAT blood analyser. Though stable at 4 litres per minute (LPM) of oxygen, we proactively secured an in-flight supply for 6 LPM to ensure a robust safety margin for emergencies. Furthermore, through close clinical coordination with the discharging hospital, we meticulously prepared a specialised medical inventory, including critical medications such as rocuronium, lorazepam, noradrenaline, and mannitol.

Because the local area could not accommodate a commercial stretcher flight, our Medical Director formulated a multi-modal strategy: transport via a cross-border ground ambulance from Croatia to Frankfurt, Germany, then transition to a long-haul commercial stretcher flight bound for South Korea.

From bedside handover to final admission, an elite team of a boardcertified emergency medicine specialist and an experienced ICU nurse meticulously managed the evacuation. Throughout the gruelling intercontinental journey, this dedicated team expertly managed the ventilator settings, performed necessary airway suctioning, and executed precise medication titrations at altitude, safely delivering the patient to the destination hospital.
