Transferring a vulnerable infant from Dubai to India

Patient background

A three-month-old ex-preterm baby, weighing significantly below the expected range for their corrected gestational age, required urgent medical transfer from Dubai to India. The infant, having spent the initial weeks of life in the NICU, remained highly vulnerable due to respiratory instability, low weight, and challenges in thermoregulation. Given the financial constraints faced by the family, arranging a costly air ambulance was not a feasible option. Instead, an alternative approach was carefully considered: a meticulously planned commercial airline transfer, conducted in coordination with the PICU team, the airline medical team, and the Bluedot aeromedical specialists.

Medical management and transfer preparations

Transporting a medically fragile neonate via a commercial flight posed multiple challenges, necessitating a comprehensive risk assessment and specialised medical interventions. The primary concern was ensuring a stable airway throughout the journey. A Hamilton ventilator with nCPAP support was utilised to maintain optimal respiratory function and oxygenation. Since neonates are highly susceptible to temperature fluctuations, an advanced temperature control strategy was implemented.

Geothermal warmer beds and extremity warmers were deployed to maintain a stable body temperature, and continuous temperature monitoring was conducted to promptly detect and prevent hypothermic episodes.

Positioning and security during the flight were equally critical. To prevent undue movement and potential complications, an airline-compatible paediatric five-point stretcher restraint system was used, ensuring safe immobilisation while keeping the baby comfortable. Given the infant’s delicate skin integrity and low body weight, specialised gel beds, designed to evenly distribute pressure, were employed to provide additional support and protection. Hydration and nutritional stability were maintained through a controlled infusion of fluids using syringe pumps, minimising the risk of dehydration and electrolyte imbalances.

The feasibility study also considered the logistical aspects of the transfer, including oxygen supply coordination with the airline, pre-flight briefings for the flight crew regarding emergency protocols, and contingency planning for any in-flight deterioration. A trained paediatric aeromedical team was present throughout the journey to monitor vital signs, adjust ventilatory settings as required, and address any emerging medical concerns promptly.

Outcome and impact

The careful planning and seamless execution of the medical transfer ensured a safe and successful journey for the infant. The neonate remained stable throughout the flight, with no significant fluctuations in vital parameters. Continuous temperature regulation prevented any episodes of hypothermia, and the nCPAP support effectively maintained respiratory function without complications. The secure restraint system provided both safety and comfort, minimising the risk of displacement or undue stress during the flight.

This case highlights the importance of strategic aeromedical planning in overcoming financial and logistical challenges associated with neonatal transport.

By leveraging available commercial flight resources and integrating advanced neonatal care solutions, the Bluedot aeromedical team enabled a cost-effective yet highly safe alternative to traditional air ambulance services. The success of this mission not only ensured the safe repatriation of the infant to India but also underscored the viability of commercial airline transfers for select neonatal patients when executed with precision and expert medical oversight.
