Johannes Hoischen, Head of Malteser AeroMedical, tells us about a biking trip to Scotland that went wrong, and how the Malteser team helped to get the patient home

Freshly graduated and standing on the threshold of his professional life, Peter, a young traveller from Hanover, Germany, set out on a long planned mountain biking journey to the Scottish Highlands. The destination – Achnashellach – was chosen deliberately. Known for its demanding trails and remote terrain, it promised one last challenge before the responsibilities of working life began.

The accident

The trip was organised by a professional tour operator and included several technically advanced downhill sections, far from populated areas and road access. During one such descent, on a rocky and exposed stretch of trail, Peter lost control of his bike. He was thrown over the handlebars and tumbled down a steep hillside, coming to rest unconscious and bleeding from multiple injuries.

His guide reached him within moments. Peter’s airbag helmet had deployed – an intervention that almost certainly prevented fatal head trauma. Nevertheless, his condition was immediately recognised as critical. Emergency services were activated without delay.

The rescue

Due to the remoteness of the location, helicopter rescue was the only viable option. On the scene, Peter was immobilised, provided oxygen, and stabilised before being airlifted to the nearest trauma hospital. At the same time, his family in Germany was informed – beginning the familiar but difficult phase of managing a serious medical emergency far from home.

Hospital diagnostics revealed the extent of the injuries: a spinal fracture, multiple serial rib fractures, and a severe concussion. While initial imaging ruled out major intracranial bleeding, Peter’s condition remained unstable. The rib fractures posed a significant risk to respiratory function, and the concussion required continuous neurological monitoring. Emergency surgery was performed, followed by admission to the intensive care unit for close observation and stabilisation.

As medical treatment began, Peter’s family contacted his travel insurer. The insurer’s assistance partner immediately activated a local agent in Scotland to provide on-site support, coordinate communication with the hospital, and ensure that Peter’s family was kept in the loop.

Going home

Once Peter’s condition began to stabilise and transport home became a realistic option, Malteser AeroMedical was appointed to manage the medical repatriation to Germany.

From the outset, the focus was on risk assessment and mitigation. Severe concussions carry the possibility of delayed neurological deterioration, particularly under the physiological stresses of repatriation. Multiple rib fractures increase the risk of complications such as pneumothorax, which can be life threatening at altitude.

It was only after all parties involved confirmed neurological stability and ruled out pulmonary complications that Peter was declared fit to fly.

A medically configured Learjet 45, staffed with Malteser AeroMedical’s highly specialised team, was dispatched. Upon arrival in Scotland, the crew conducted a personal bedside assessment, reviewed all imaging and reports, and coordinated directly with the hospital team. Following confirmation, Peter was transferred via ground ambulance from the Scottish hospital to the nearest airport.

The flight to Germany proceeded without complications, with ongoing monitoring of respiratory and neurological status throughout. After landing, Peter was handed over seamlessly to a hospital in Hanover, where further surgical care and rehabilitation were initiated.

Peter’s recovery will require time and structured rehabilitation, but his prognosis is positive and he remains committed to returning to Scotland to finish the exact trail that caused this unfortunate adventure.
