Michael Cooke, Founder and CEO of Avcair and Medcair, speaks to ITIJ about his new business, family legacy, and what’s next

You now have two businesses: Avcair and Medcair. You launched Avcair almost 25 years ago – what was the impetus in launching that business initially?

When I launched Avcair nearly 25 years ago, the impetus was both deeply personal and entrepreneurial. Aviation runs through my family’s veins. I am a fourth-generation pilot, and I grew up around the industry with a love for flying and a belief in what it makes possible. But beyond the passion, I saw a gap in the market for a more personalised, flexible, and client-focused aviation service.

At the time, private aviation in Australia was often transactional and rigid, and I wanted to build something different: a company that not only delivered safe and efficient operations but also placed relationships and tailored service at the centre. I started Avcair with no money, no formal business background, but with a vision to create an aviation business that felt more like a partner to its clients than just a provider. That foundation of passion, service, and resilience has allowed Avcair to grow from a single aircraft operation into a recognised international aviation group.

How did your family heritage in both aviation and medicine naturally lead to the creation of Medcair?

My family has always stood at the intersection of aviation and medicine. On one side, I am a fourthgeneration pilot with a lineage that dates back to the earliest days of flight in Australia. On the other, my father dedicated his life to medicine, earning national honours for his service. Growing up, I watched both fields in action: one focused on breaking boundaries of distance, the other on preserving and improving life.

Medcair was born as a natural fusion of those two legacies. Aviation gave me the platform to move quickly and connect people across borders, while medicine gave me a profound sense of purpose: to use that reach in service of human wellbeing. Combining the two was never just a business decision, it was a calling. Medcair is about leveraging world-class aviation infrastructure with the highest medical standards, to ensure people receive the best possible care wherever they are in the world. In many ways, it feels less like launching a new company and more like continuing a family tradition of service through innovation.

Medcair is committed to, and passionate about, medical aviation services. What inspires that passion and motivation?

Our passion for medical aviation comes from the belief that what we do has an immediate, life-changing impact. In traditional aviation, you are moving people or assets from point A to B. In medical aviation, every mission carries a human story: someone’s parent, child, or loved one who needs urgent care. That responsibility inspires us every day.

I have seen firsthand how the combination of aviation and medicine can be the difference between life and loss. When a patient is thousands of miles from home, injured or critically ill, the ability to bring them safely back to their family and into the right medical hands is incredibly powerful. That is what motivates us: knowing that our expertise in aviation is not just about logistics, it is about delivering hope.

At Medcair, our team shares this sense of purpose. We do not see it as just a service; we see it as a calling. The passion comes from knowing that every flight we operate could be the most important journey of someone’s life.

What is next for Medcair, and how will your foundational values continue to guide the growth of the business?

The next phase for Medcair is about scale, innovation, and reach, but always anchored in the values that shaped us from day one. We are expanding our global network so we can respond more rapidly and more reliably, no matter where a patient is located. That means investing in new aircraft capability, strengthening our medical teams, and building partnerships with insurers, assistance providers, and hospitals worldwide.

At the same time, technology will play an increasing role. We see opportunities to integrate telemedicine and digital health tools into our operations so that care begins before a patient even boards one of our flights. The future of medical aviation is not just about transport, it is about continuity of care across borders.

Through all of this, our foundational values remain constant: safety, service, compassion, and excellence. These principles guided the creation of Medcair, and they ensure that as we grow, we never lose sight of why we exist: to provide the highest standard of care and to bring peace of mind to families when they need it most.
