Medical repatriation of a complex patient with a multiple diagnostic history

Marm Assistance recounts how they repatriated an elderly patient with multiple comorbidities from Frankfurt, Germany, to Saint Petersburg, Russia

Although air travel is usually safe, transporting a patient with multiple comorbidities requires precise planning and continuous supervision. This case study details the repatriation of an 88-year-old woman with a history of non-traumatic intracranial haemorrhage, epilepsy, and bronchopneumonia. She was transferred from Frankfurt to Saint Petersburg via Istanbul under close medical supervision.

At the time of transport, the patient was stable, though she continued to require rehabilitation and regular medication. The journey lasted approximately 11 hours, including a layover. Each stage was carefully planned to anticipate potential complications, and ensure continuity of care.

Preparation

Marm Assistance coordinated the mission following a referral from a partner company. Dr Batyr Kurbanov was assigned as the escort physician. Fluent in both English and Russian, he was able to communicate with the patient in her native language, providing reassurance and comfort.

Before departure, the latest clinical reports were reviewed. A bespoke medical plan was prepared, covering medication schedules, stabilisation equipment, and in-flight management. Ground ambulances were arranged at both ends of the route. Marm Travel organised commercial stretcher flights, while Marm Medical Operations ensured seamless coordination.

Repatriation process

The patient was taken onboard by stretcher and ambulift. Her vital signs were stable at take-off, and the seat belt was secured. Once airborne, however, she attempted to unfasten the belt and sit up several times. She was reassured, the restraints were readjusted, and a mild sedative was administered. This allowed her to rest calmly while vital signs were monitored at regular intervals.

During the layover in Istanbul, Turkey, the patient remained in the airport medical area under constant supervision. This constituted a monitored waiting period rather than a formal hospital admission. Meals were provided, medications were administered on time, and clinical parameters were observed.

For the second flight, she was again transferred by stretcher. Shortly after take-off, she tried to stand up. The physician explained the risks and administered another sedative. She calmed down, slept through most of the flight, and remained stable until descent.

Upon arrival in Saint Petersburg, she was handed over to the waiting ambulance team on a stretcher. A complete medical report and in-flight summary were delivered to the receiving hospital staff.

Conclusion

This case highlights the importance of detailed preparation, robust logistics, and vigilance in medical repatriation. Despite challenges such as patient restlessness and the long journey, the mission was completed without incident.

A thorough pre-flight assessment, tailored medical support, and effective coordination between operational and medical teams ensured a safe outcome. The presence of an experienced physician able to adapt quickly to in-flight events was critical to the mission’s success.

This case demonstrates that patients with complex medical histories can be repatriated securely when preparation and teamwork are prioritised.
