Stanley Yoder of Phoenix Air on the elite group of medevac professionals the world turns to when no-one’s coming

In June 2017, the world watched as one of the most complex medical evacuations in modern history unfolded – the return of Otto Warmbier from North Korea to the US.

Seventeen months earlier, the 21-yearold college student had been sentenced to 15 years of hard labour by North Korea’s Supreme Court. Following extensive diplomatic negotiations, his release was finally secured. Yet one critical question remained – who could safely bring him home under such unprecedented circumstances?

Phoenix Air Group was tasked by the US Department of State to execute the operation.

A highly modified cargo-door Gulfstream air ambulance, call sign ‘Gray Bird’, was assigned to the mission. Configured to carry up to 15 passengers or five full-size medical stretchers, this Phoenix Air aircraft transported both the medical team and US government delegation while serving as a fully integrated mobile intensive care unit for the long journey between Pyongyang and Ohio.

Into the unknown

Information regarding Otto’s medical condition was limited and vague. Communications were restricted, and access tightly controlled.

The Gulfstream departed Cartersville, Georgia, with an emergency medicine physician and two critical care flight nurses on board, prepared for any scenario – both clinical and non-clinical. The aircraft carried an extensive formulary of medications and state-of-the-art equipment, including transport ventilators, invasive monitoring capabilities, infusion pumps, wireless ultrasound, and handheld blood gas analysers. Redundant equipment, power systems, and extended-duration oxygen supplies were essential, as there would be no opportunity to divert.

After a stop in Washington, D.C., the crew continued west, staging in Japan before crossing into North Korean airspace. The flight crew later recalled the moment they realised they had passed the point of no return.

Welcome home, Otto

After arriving in Pyongyang, Dr Mike Flueckiger of Phoenix Air was escorted to the hospital and permitted to examine Otto. Following final judicial proceedings, Otto was released into Phoenix Air’s care. The medical team stabilised him, transported him to the aircraft, loaded him through the cargo door, and departed without incident.

Hours later, as the aircraft touched down on American soil, a flight nurse knelt beside him and quietly whispered, “Welcome home, Otto.”

Otto was transferred to an awaiting medical team. As many recall, though his condition proved irreversible, the mission achieved what mattered most – bringing Otto home to his family.
