Dr Manu Krishna, Chief Operating Officer at Aeromedical & Marine TRI, talks to ITIJ about connecting expertise, logistics and care worldwide

In an increasingly interconnected world, international medical assistance has evolved far beyond simply transporting a patient from one location to another. Every mission requires clinical expertise, operational precision, regulatory knowledge and seamless coordination across borders. At Aeromedical & Marine Training & Rescue International (Aeromedical & Marine TRI), this philosophy has been embedded in the organisation from its foundation.

The vision

The company was founded by Dr Monica Fortea Garcia, an experienced orthopaedic and trauma surgeon whose career has extended from hospital medicine and surgery to helicopter emergency medical services (HEMS), fixed-wing aeromedical operations, international patient repatriation and medical education. After more than two decades working with patients in the air and on the ground, and teaching future aeromedical physicians and nurses, Dr Fortea envisioned an organisation where clinical decisionmaking and operational expertise would work together under one international network.

Today, that vision has developed into a global medical assistance organisation headquartered in Barcelona, Spain, with operational hubs in Miami, Dubai and Singapore. This international footprint enables Aeromedical & Marine TRI to coordinate medical assistance across Europe, the Americas, the Middle East, Africa and Asia Pacific, while maintaining 24/7 operational capability.

The offering

The company’s services encompass air ambulance evacuations and repatriations, commercial airline medical escorts, commercial stretcher transfers, maritime and offshore medical assistance, worldwide ground ambulance coordination, critical care transport, paediatric and neonatal transfers, ECMO-supported missions, VIP medical escorts, and international hospital coordination and case management.

Yet the complexity of international medical transport lies behind the journey itself. A successful repatriation may involve clinical assessment, fit-to-fly evaluation, airline medical clearance, specialised equipment, oxygen requirements, airport and tarmac coordination, ground ambulances, receivinghospital admission and continuous communication across several countries and time zones.

Aeromedical & Marine TRI therefore approaches each mission as an end-to-end clinical and operational pathway. Its multidisciplinary network of physicians, nurses, paramedics, case managers and operational specialists works alongside insurers, assistance companies, hospitals, airlines and international partners to maintain continuity of care from bedside to bedside.

This integrated approach becomes particularly important in complex missions. From long-haul critical care repatriations and trauma cases to neonatal, paediatric and highly specialised transfers, every operation is individually planned according to the patient’s clinical condition, mobility, destination and logistical requirements.

Strengthening knowledge

Education is equally central to Dr Fortea’s original vision. Aeromedical & Marine TRI contributes to professional development through training in HEMS, fixed-wing air ambulance operations, medical escort operations, maritime medicine and critical care transport. The objective extends beyond completing today’s missions: it is also about strengthening the knowledge and standards of the professionals who will deliver aeromedical care tomorrow.

The company’s expansion into Singapore represents the latest stage of this international development, strengthening its ability to support partners throughout Asia Pacific alongside established capabilities in Europe, the Americas and the Middle East.

As healthcare and travel become increasingly global, successful medical assistance will depend on trusted networks, rapid communication and strong partnerships. Aeromedical & Marine TRI continues to grow around the principle on which Dr Fortea built the organisation: combining medical expertise with operational precision, while never losing sight of the person at the centre of every mission.

Global reach. Medical expertise. Human commitment.

This month in ITIJ we take a close look at how travel insurers are responding to the changes in winter sports risk, and we ask how risk is priced, covered and communicated to policy holders.

How do international hospitals stand out from the crowd, and become the number one choice for insurers and assistance companies? Clinical excellence alone is no longer enough. We look at why hospital culture has become a strategic advantage, and how dedicated international departments are strengthening relationships between hospitals, insurers, and assistance companies.
