Urgent repatriation for a leukemia patient from a remote Atlantic island to Asia

When a guest worker living on a remote island in the Atlantic was diagnosed with acute leukemia, local healthcare resources were unable to provide the tertiary hematology-oncology services required, making an overseas transfer essential.

The options

Upon receiving the referral, Allshores Assistance initially explored treatment options in North America and Europe. However, the patient did not hold visas for the countries under consideration and obtaining medical treatment visas would take longer than the patient's clinical condition allowed.

At the same time, a clinical review determined that commercial air travel was not appropriate due to the patient's medical status. Air ambulance transport would be required. This led to a further complication: the annual policy maximum was unlikely to cover both long-distance air ambulance transport and the extensive specialist treatment anticipated in traditional referral destinations.

The assistance team re-evaluated available options and decided repatriation to the patient's home country in Asia was the most clinically appropriate and financially sustainable alternative. It ensured access to specialist treatment and provided the support of family and friends during what was expected to be a prolonged treatment journey.

The repatriation

Allshores Assistance engaged several air ambulance providers to identify a partner capable of completing an urgent longhaul transfer. Latitude Air Ambulance was selected based on its capabilities and immediate availability.

Simultaneously, potential receiving hospitals in the patient's home country were identified and coordination began between family members, physicians, social workers, transport specialists and case managers across multiple countries and time zones.

Securing an accepting hospital, however, proved complex. Receiving facilities advised that an accepting hematologist-oncologist first needed to review and agree to manage the case before a bed could be offered. The assistance team worked closely with the patient's family and transport provider to identify suitable specialists and facilitate discussions.

Billing challenges

After several days of collaboration, an accepting physician was secured, but the receiving hospital advised that a local representative would be required to facilitate guarantees of payment and direct billing arrangements.

Leveraging in-house language capabilities, Allshores Assistance communicated directly with hospital administrators in their native language, addressing concerns. As a result, the hospital agreed to establish direct billing arrangements without requiring a local intermediary.

Attention then turned to the transfer itself. Medical records were shared while the sending hospital optimised the patient's condition for transport. A final obstacle arose when the proposed air ambulance routing raised questions regarding international transit requirements. Working closely with transport partners and relevant authorities, the team clarified requirements and identified a compliant routing solution, avoiding further delays.

A successful transfer

Ten days after the initial referral, the air ambulance crew assumed bedside care and the patient was successfully repatriated to Asia under continuous medical supervision. Upon arrival, the patient was admitted directly to the receiving hospital and care was formally transferred to the local medical team.

This case demonstrates how creative thinking, international coordination and a willingness to challenge traditional referral pathways can transform a complex assistance case into a successful patient outcome. It also highlights the value of repatriation as a strategic care solution that balances clinical, financial and personal considerations. Through close collaboration and persistent problem solving, the team overcame significant barriers to secure timely treatment and a successful transfer for the patient.
