Coordinating a complex medical repatriation from Iraq to Germany
When a patient was hospitalised in Iraq, CONNEX Assistance found that the case was more complicated than it initially seemed. Lara Helmi reports
When CONNEX Assistance was notified by a major insurer of a patient hospitalised in Baghdad, Iraq, the case initially appeared to be a routine request for medical information. The patient had been admitted with chest pain and a persistent cough, and the insurer asked CONNEX Assistance to obtain a medical report and assess whether the treatment being provided locally was adequate. What followed, however, evolved into a complex medical repatriation that required rapid coordination, medical oversight, and logistical problem‑solving across multiple countries.
Finding the patient
One of the first challenges was simply locating the patient. The hospital in Baghdad operated five different branches, and communication proved difficult. Administrative offices were closed for the weekend, and contact with hospital staff was limited. Despite these obstacles, the CONNEX Assistance team successfully identified the patient’s location and established communication with the treating physicians.
The patient’s condition deteriorated further, with worsening respiratory symptoms and difficulty breathing. Based on the clinical updates, the CONNEX medical team advised the insurer that a medical evacuation might soon be required. Further examinations found the initial diagnosis was likely incorrect. Additional testing was conducted while the patient was unfit to travel, while the doctors investigated the underlying condition.
A diagnosis and approval to fly
Approximately a week later, the treating physician confirmed a diagnosis of lung cancer and declared the patient fit to fly. The patient expressed a strong desire to return home to Germany. Following discussions with the insurer, CONNEX Assistance was asked to arrange a medical repatriation with a physician escort and oxygen support for the journey from Baghdad to Germany.
Preparations began immediately. The selected escort physician held a New Zealand passport, and the team verified that a visa could be obtained quickly. Flights, medical equipment, and ground transportation were coordinated across multiple locations. Services arranged by CONNEX Assistance included commercial flight tickets for the patient and escort, oxygen on board the aircraft, a portable oxygen concentrator, escort accommodation, ambulances in Baghdad and Germany, airport assistance, security escort to Baghdad airport, and all necessary travel documentation.
Ready for departure
Upon arrival in Baghdad, the escort physician conducted a bedside assessment and found the patient’s condition had worsened compared with the most recent report. The patient had developed fever and increased respiratory distress. Oxygen requirements had risen to 4–8 L/min, while the airline could only provide 4 L/min. The physician therefore planned to use the portable oxygen concentrator with him to supplement the flow.
To minimise physical strain on the patient, CONNEX Assistance coordinated special airport permissions allowing direct ambulance access to the aircraft on the tarmac.
Delayed in Cairo
However, the journey presented further challenges. During the transfer in Cairo, airport medical quarantine required the patient to undergo an additional medical examination. The airport doctor initially declared the patient unfit to continue the journey. After reassessment the patient was cleared to travel, but the delay caused the team to miss their connecting flight.
CONNEX Assistance responded immediately, arranging admission to a hospital near Cairo Airport and securing the next available flight with oxygen support for the following morning. The patient was safely transported to the airport the next day and completed the journey to Germany with the escort physician.
Upon arrival, a pre-arranged road ambulance transferred the patient directly to a hospital for continued treatment. Despite numerous operational and medical challenges, the CONNEX Assistance platform successfully coordinated and executed a complex international medical repatriation, ensuring the patient could return home safely for further care.
