Bernadette Breton, Founder of AIMS, speaks to ITIJ about their important partnerships, fraud challenges, and what’s next for the business

Q. You have worked in the assistance industry for almost 23 years. What have been your biggest successes and challenges to date?

Over the past 23 years in the assistance industry, one of my biggest successes has been building Alliance International Medical Services (AIMS) into a trusted provider across Africa. Establishing a network of reliable hospitals and medical professionals throughout the continent has been incredibly rewarding. We've helped thousands of patients, sometimes under very challenging circumstances, and being able to deliver timely, life-saving care is something I’m deeply proud of.

However, the journey hasn't been without challenges. One of the biggest hurdles has been navigating the complexities of healthcare in different regions – varying standards, infrastructure limitations, and financial constraints together with the awareness of both fraud and corruption, all create difficulties where billing and ethical practices can sometimes be compromised.

We've had to be proactive in combating this through audits and strong partnerships. But through it all, the dedication of AIMS colleagues and the relationships we’ve built with local providers have been key to overcoming these challenges and achieving the success we see today.

Q. What does your current role involve?

As the CEO, my role involves overseeing the strategic direction and overall operations of the company. I endeavour to ensure that AIMS continues to provide high-quality medical assistance services across Africa, manage partnerships with hospitals and service providers, and to lead the team to deliver reliable support to clients. My responsibilities also include guiding the company's growth, maintaining industry standards, and exploring new opportunities to expand AIMS' services. Additionally, I stay engaged with developments in the medical field to enhance the company’s offerings.

Q. You are based in South Africa. How important is your location for the business?

Being based in South Africa is incredibly important for AIMS. The location places us at the heart of the African continent, where the need for reliable and professional medical assistance is significant. South Africa's advanced healthcare infrastructure and strong network of hospitals and providers make it an ideal hub for coordinating services across the region. This strategic position allows AIMS to respond quickly to medical emergencies, facilitate evacuations, and work closely with local and international partners to ensure seamless medical care throughout Africa. It also strengthens AIMS' connection to the continent’s unique healthcare challenges and needs.
 

Q. You have partnerships with major hospitals on the African continent. Tell us why these relationships are so vital.

The partnerships that Alliance International Medical Services (AIMS) has with major hospitals across the African continent are vital for several reasons. Collaborating with reputable hospitals ensures that AIMS can offer clients access to high-quality medical care, which is essential for effective treatment and patient outcomes. Strong relationships with hospitals facilitate quick coordination during medical emergencies, enabling AIMS to respond promptly and efficiently to patient needs, whether for evacuation or direct medical assistance. These partnerships allow for the sharing of resources, knowledge, and best practices, enhancing the overall quality of care provided and improving operational efficiency.

Local partnerships provide insights into regional healthcare practices and cultural sensitivities, helping AIMS tailor its services to meet the specific needs of different communities. Overall, these relationships are essential for AIMS to deliver comprehensive, realistic, knowledgeable and effective medical assistance services across Africa.

Q. As tourism grows, do you think there is capacity for more assistance companies to enter the market?

As tourism continues to grow in Africa, there is room for increased collaboration within existing service providers within this space. With more travellers comes a greater need for reliable medical support, and expansion of existing experienced companies can help meet that demand. This growth can also foster healthy competition, encouraging all providers to enhance their services.
 

Q. There are challenges around currency payments for medical care. Insurers prefer to pay in local currency, whilst the hospitals would prefer dollars. What are your thoughts around this issue?

The issue of currency payments in medical care can be a challenge, particularly outside of South Africa, and this is something we navigate regularly at AIMS. Hospitals in many of the African regions prefer payments in dollars owing to exchange rate fluctuations and the instability of the local currency, and the availability of the dollar itself specifically, and of late, in francophone regions, the availability of the euro. On the other hand, insurers prefer local currency as it’s often more practical and cost-effective for them.

The key here is balance and flexibility. At AIMS we work closely with insurers, hospitals and service providers to find a solution that works for everyone. Sometimes this involves negotiating terms that protect both parties from exchange rate risks and finding alternative payment structures that satisfy all stakeholders, whilst adhering to all local governmental currency laws. Ultimately, our goal is to ensure seamless care for the patient, while managing the financial aspect in a way that’s fair and transparent.

Building strong relationships with trusted medical providers is crucial

Q. You spoke at ITIC MEA in Cairo in April about fraud, and said the common practices include inaccurate billing, upcoding, phantom billing and under-the-table gratuities. What can staff do to combat these issues?

To combat fraud, it’s essential for staff to stay vigilant and follow strict protocols. First, thorough training is key and staff should be well-versed in spotting red flags like unusual billing patterns or discrepancies in medical records. Regular audits and cross-checking invoices can also help detect upcoding and phantom billing early on.

Building strong relationships with trusted medical providers is crucial. This ensures transparency and discourages unethical practices. Staff should also feel empowered to report any suspicious activities without fear of repercussions – fostering a culture of integrity.

Finally, clear communication with insurers and clients about what is and isn’t covered ensures there are no misunderstandings or opportunities for fraudulent claims. By staying informed, proactive, and communicative within the industry, together we could significantly reduce these fraudulent practices.

Q. What’s next for AIMS?

At Alliance International Medical Services (AIMS), we're focused on expanding our reach across Africa and enhancing the quality of care we provide.

We're exploring new partnerships with hospitals and service providers to strengthen our network and continue offering reliable medical assistance in even the most remote areas.

Additionally, we're investing in technology to improve efficiency, from faster response times to seamless coordination with insurers.

As tourism grows and the healthcare landscape evolves, we’re committed to staying ahead by offering innovative, patient-centred solutions. We’re also focusing on capacity-building within our team, ensuring our staff are equipped with the latest skills and knowledge to handle complex medical cases. Bernadette Breton, Founder of AIMS
Bernadette’s career sent her to South Africa in 1982, on a two year medical contract where she worked at the then Johannesburg General Hospital – 2,000 beds divided into five different colorful blocks now known as Charlotte Maxeke. Later she managed an ICU within what became The Netcare Group of Hospitals. It was during this time that Bernadette discovered her passion for medical assistance, which ultimately led her to create AIMS – now a trusted provider of medical assistance services across Africa.
